Our dedicated case managers promote personal and community independence through their skills, care, empathy, and dedication to reaching a larger demographic.
At Frontiers Wellness Services, we consider our service recipients as our partners. Our goal is to provide them with multi-facilitated support that will teach them the required skills to gradually reduce their dependence on our services.
FWS’ dedicated case managers connect them with a range of support, including mental health, medical care, housing services, education, transportation, and employment opportunities to enhance their everyday independence.
